Below are a number of documentation and billing templates I created and use at my home institution. They are all fillable templates and work best with Adobe Reader. They were first created with Adobe Acrobat Pro; I'd recommend making revisions to the templates using this application. As you'll see, I've removed all logos and other identifying information from my home institution. You can feel free to download these, change them, do whatever you'd like. I'm posting these in hopes you won't have to "reinvent the wheel." Enjoy!
